BETA Convention Permission Form

            I give my permission for my (son, daughter)____________________ to attend the Junior Beta Convention at the Opryland Hotel in Nashville, Tennessee on November 20-22. I understand that he or will be traveling by school bus and will be under the care of Mr. Bright, Mrs. Griffin, and Mrs.Vogt, Mr. West, and several chaperones. I also understand that these sponsors, principal, and chaperones will take the utmost care with my child and do not hold them responsible for anything out of their control.

            My child and I have read the CONDUCT CODES, PAID ANY EXTRA FEES, and agree to abide by the code of conduct and/or any decisions that have to be made by the sponsors and realize that any departure from such will be grounds for dismissal from the club as well as being sent home.
